Continuity: Graham and Anne's hands when they discuss therapy in the cafe.
Continuity: The amount of ice tea in Anne's glass when she first visits Graham's house. He hands her a glass about half full and then later we see that it is filled almost completely when she almost spills it.
Continuity: When John watches Anne's tape, it doesn't match what we saw when she recorded it.
Audio/visual unsynchronized: On the first "video" we see Graham watching, the sound of a film projector is heard as a background to the woman's voice. Video doesn't make this sound.
Continuity: The view from John's office window is different at the end of the film
Continuity: When watching the videos the camera is constantly moving as if was hand-held during the interviews. In almost every interview you see Graham set the camera down before starting their discussions.
Continuity: When Graham is interviewing Ann, Ann sets the camera down on the arm of the chair pointing at the window away from the couch. When Graham gets up to turn it off, it is pointing at the couch.
Continuity: When John bursts into Graham's apartment, he finds Cynthia's and Ann's tapes in the boxes on the table. Both tapes have the same date on them, even though it is clearly implied that several days have past since Cynthia made her tape.
Continuity: When Anne comes to Graham's apartment to make the tape, she rests on the side of a doorway with her hand in her back pocket. In the next shot, her hand is in her front pocket.
Continuity: Ann's necklace when she's in bed with her husband, asking him if he's having an affair.
Audio/visual unsynchronized: During the scene where Ann asks John if he is having an affair, we hear Ann say "alright, alright you've made your point," but her lips move out of time with what is said.
