Also to add, Elliot goes out with a guy who works at Sea World for a period of time. We have already figured out that Scrubs takes place in California, so with this information San Diego, Ca, looks pretty good because the only Sea World in Cali is in San Diego.

-Edit- We do know that Elliot is from Connecticut, JD is from Trotwood, Ohio, and that Carla is from Chicago. Out of these 3 character birth locations, it could be in any one, or none of them.

::Edit:: In Eugene, OR there is a Multi-Care hospital complete with mental health facilities, pediatrics, a top notch ER, and everything else (other than the characters themselves) that are consistent with the Sacred Heart in the show. Whereas in California, there is not such a close match. But given the details of the show: Seaworld, the often-good weather, the area codes on the phone numbers, etc. it is probably meant to be in California.

The episode was originally positioned between My Manhood and My Dumb Luck. Due to the writers strike, the season was cut short and the NBC executives probably chose this episode for a decent finale...that explains the continuity problem.
